Employment Community First Direct Support Professional

The person in this position is responsible for providing direct support for individuals in Waves’ Community Day services. The position is under the supervision of the Employment and Community First (ECF) Program Manager and corresponding Case Manager.

Direct Support Professional

In conjunction with Residential Program members and in alignment with the agency mission and team goals, the person in this position is responsible for providing support services to adults with disabilities. This position is under the supervision of the Residential Program Manager.

Franklin Day Program Coordinator

Under the general supervision of the Franklin Day Program Manager, the person in this position is responsible for coordination, development, implementation, and documentation of services provided to service recipients served by the Franklin Day Program, Semi-Independent Program, and VR Employment Program (all services and programs referenced throughout as Franklin Day Services). This may include, but is not limited to, employment-related services, ensuring smooth operations of the Franklin Day Program, management of service recipient records, management of billing and attendance related records, participation in planning and COS meetings, fostering community relationships in an effort to expand opportunities for service recipients, direct support for service recipients as needed.

Franklin Day Direct Support Professional

In conjunction with the Day Program members and in alignment with the agency mission and objectives, the person in this position is responsible for providing support to consumers in the Day Program. The position is under the supervision of the Day Program Manager.

Residential Medical Coordinator

Under the general supervision of the Franklin Residential Program Manager, the person in this position is responsible for coordination, implementation, record keeping, and general oversight of all medical services provided to persons served by the Franklin Residential Division. This may include, but is not limited to: attending and documenting medical appointments, training staff on therapy and medical plans, upkeep of medical records according to DIDD requirements, ordering medications, processing MAR’s, and participating in agency, COS and ISP meetings. The person in this position will be included in the after-hours emergency on-call rotation for the Franklin Residential Division. The person in this position is expected to be knowledgeable of all DIDD guidelines and requirements related to the services performed.
